Ambient AI Scribes — Pricing guide
Ambient AI scribe pricing guide
Ambient AI scribe pricing has collapsed from $600+ PMPM in 2023 to $150–$400 PMPM in 2026 for standalone products, and $50–$150 PMPM as an EHR add-on. Payback is under 3 months at typical primary care volumes if the scribe reliably saves 30+ minutes per clinician per day.

This category still commands premium pricing but is compressing fast. Any vendor asking for a multi-year commitment on today's price is asking you to overpay.
| Model | Typical range | Best for | Watch out |
|---|---|---|---|
| Per provider per month (standalone) | $150–$400 PMPM | Small to mid practices, month-to-month or annual. | Some vendors cap encounters per month — confirm 'unlimited' really is. |
| Per encounter | $1.50–$4.00 per completed note | Low-volume specialists, part-time providers. | Encounters can add up fast — model against PMPM equivalent. |
| EHR-embedded add-on | $50–$300 PMPM as line item on EHR bill | Practices happy with EHR vendor's native scribe. | Ties you deeper into EHR contract; less feature velocity than standalone. |
| Enterprise volume | $100–$250 PMPM at 25+ seats | MSOs, large groups. | Multi-year lock-in common — negotiate 30-day out for underperformance. |
| Pilot / free trial | $0 for 14–30 days, 3–10 providers | Every vendor should offer this — take it before signing. | Confirm data deleted after pilot ends and models not trained on your data. |
Hidden or often-overlooked costs
- EHR API integration setup: $2k–$10k one-time
- Custom template build: $500–$3,000
- Overage encounters: $1–$3 per note above included tier
- Clinician training: $200–$1,000 per provider or self-serve
- Data export on termination: often refused or expensive
Benchmark note
Peer-reviewed studies (JAMA, KLAS) show top-tier ambient scribes reduce documentation time by 50%–75% in primary care and 30%–60% in specialty. ROI at $250 PMPM breaks even at ~15 minutes saved per day at $200/hour blended provider cost.

Frequently asked questions
How much does an AI medical scribe cost?+
Ambient AI scribe pricing typically runs $99–$399 per provider per month. Lower tiers cap usage (encounters or audio minutes). Flat-rate all-you-can-use pricing exists but is trending toward tiered as vendors face real inference costs. Enterprise deals for larger groups can go below $150 per provider per month.
Do AI scribes work with my EHR?+
Most major AI scribes list integrations with every major EHR. The real question is integration depth: does the note write to the correct chart location automatically, or does the provider copy-paste? Does the scribe write structured data (medications, orders) or only free text? Test this in a real pilot, not from a screenshot.
How accurate are AI medical scribes?+
Accuracy varies sharply by specialty and by encounter type. General internal medicine visits are the sweet spot for most vendors. Specialty vocabulary (psychiatry, dermatology, ophthalmology, PT/OT), non-English patients, and multi-speaker rooms are where vendors differ. The only way to know is a 2–4 week pilot with your actual providers.
Is patient data safe with an AI scribe?+
Any legitimate vendor signs a HIPAA BAA. The additional question that matters: does the vendor use your PHI to train their models? Best practice is that PHI is not used for model training by default, with an explicit opt-in. Encryption in transit and at rest, geographic hosting, and defined retention should all be documented.
Do I need to tell patients an AI scribe is in the room?+
Consent practice varies by state law and by vendor implementation. Most vendors provide patient-facing consent language and signage. Confirm your state's recording law and your vendor's guidance. Many practices treat it like any other documentation aid and include it in general consent-to-treat paperwork.
How long does an AI scribe pilot take?+
Plan 2–4 weeks with multiple providers using the scribe on real encounters. Week 1 is workflow learning and adjustment; week 2 onward is where you can measure provider edit time per note, error rate on your specialty's vocabulary, and satisfaction. A one-week trial is a demo, not a pilot.
